Failure to file tax reports leads to Virginia license revocations
Virginia regulators recently revoked insurance licenses to a number of agents and companies that failed to file a 2008 Annual Gross Premiums Tax Report on time, as required by Virginia law.
Virginia State Corporation Commission Bureau of Insurance records indicate each person or entity had the agent and surplus broker license revoked for one year, at which time application can be made for reinstatement.
Those whose licenses were revoked include:
• Charles McCloskey Jr., of Ridgefield Park, N.J.
(Case No. INS-2009-00110).
• John G.B. Allen, Woodbury, Minn.
(Case No. INS-2009-00111).
• Apex Partners Holdings LLC of Dallas, Texas
(Case No. INS-2009-00112).
• CJD & Associates LLC, of Overland Park, Kan.
(Case No. INS-2009-00114).
• Firestone Insurance Agency of Virginia Inc., of Williamsburg, Va.
(Case No. INS-2009-00115).
• Layline Risk Management Partners LLC, of Hunt Valley, Md.
(Case No. INS-2009-00116).
• Turner Surety and Insurance Brokerage Inc. of Woodcliff Lake, N.J.
(Case No. INS-2009-00117).
• Yearsley Bloodstock Insurance Services (Lexington) Inc., of Lexington, Ky.
(Case No. INS-2009-00118).
