Man uses false insurance applications to get $3,700 in commissions
A broker-dealer from New York was barred from associating with FINRA members after he submitted false insurance applications for fictitious customers, leading to him receiving $3,379,28 in commissions from an insurance company, FINRA records show.
John Joseph Mazzella, a registered representative from Yonkers, N.Y., submitted the false insurance applications and a false preliminary insurance application for a customer without their authorization or consent, according to FINRA investigators.
Mazzella also failed to appear for an interview with FINRA investigators.
