Pennsylvania insurance agent’s license revoked over annuity filing
A life insurance agent has lost his license after using his mother’s name and address on a client’s application for an annuity, then tious annuity application has had his insurance license revoked.
The Pennsylvania Insurance Department took the action against Steven Oravitz of New Brighton, Pa., in April for misappropriating premium funds and fraud, according to recently released department records.
PID investigators found that Oravitz caused his mother’s name and home address to be on an annuity policy application of a client in October 2005. He identified his mother as the sister of the client, which she was not, and as the owner of the annuity on the application. He caused the client’s annuity policy application to be cancelled before a policy was issued, resulting in a refund in the initial premium submitted with the application, records show.
The refund check for $5,544.75 was mailed to his mother’s address, based on the false information in the application. He deposited the premium refund check into his personal account.
Oravitz told investigators that he paid back the client, but has no proof of the transaction, records show.
Western & Southern Life Insurance Co. terminated him with case for his actions in connection with the annuity transaction and for not complying with company fiduciary standards, records indicate.
Investigators say he also failed to notify the department of a new business address within 30 days, as required by state law.
He agreed to the license revocation, according to PID records.
