Health Net Inc. is investigating the apparent loss of several server drives containing employee, member and health care provider information from its data center in Rancho Cordova, Calif.

IBM, the company’s vendor that manages its information technology infrastructure, reported that it could not local several server drives, according to the Los Angeles, Calif.-based national health insurer.

A statement from Health Net, which provides health insurance to about 6 million people across the U.S., did not indicate when the notification from IBM came or how many people’s personal information may have been compromised.

The statement also did not indicate if it knew which of its insurance programs may have been compromised. Health Net provides group, individual, Medicare, Medicare Part D, Medicaid, Department of Defense, including TRICARE, and Veterans Affairs health insurance programs.

After a forensic analysis, Health Net has determined that personal information of some former and current Health Net members, employees and health care providers is on the drives, and may include names, addresses, health information, Social Security numbers and/or financial information, according to Health Net’s statement.

The company is continuing an investigation, but it chose to notify people whose information was on the drives “out of an abundance of caution,” according to the statement.

The insurer is offering affected people two years of free credit-monitoring services, which will include fraud resolution and, if necessary, restoration of credit files, as well as identity theft insurance. These services will be provided through the Debix Identity Protection Network.
